This 15-question survey is an awesome first week of school activity for students to complete in class or even as a first week take-home assignment.

In this resource you will find:

- A ready-to-print version to download and send straight to the printer

- An editable version that gives you access to edit every question

- A google form link to make a copy of the digital survey (so if you are ready to rock and roll with google classroom and other learning management systems early on--this version is for you!)

The suggested survey questions are suggestions, however I have curated the question set to intentionally engage students in sharing about what they stand for, their values and their relationship and attitude toward school and learning.

This survey is a great way to start off honoring student identity and voice and it supports teachers in preparing for the school year!
